About Prosomio

Practise the consultation, not just the medicine.

Prosomio is a UK clinical scenario simulator for GP trainees, medical students, trainers and training programmes. Talk to AI patients, role-play with colleagues and get structured feedback on how you consult, not only on what you know.

Created by practising GPs and GP trainers

Sample feedback (illustrative)

  • Data gathering and diagnosis

    Explored ideas and concerns early

  • Clinical management and medical complexity

    Safety-netting could be more specific

  • Relating to others

    Clear explanation in plain language

Our mission

Make good consultation practice available to everyone who needs it.

Knowing the right answer is only half of general practice. The other half happens in the room: listening for the cue, explaining a plan in words a patient can use, sharing a decision, and closing safely in twelve minutes.

Our mission is to give every learner plenty of realistic, low-stakes chances to practise that half. They should be able to do it whenever suits them, with feedback that is specific enough to act on.

“Consultation skills grow through repetition and reflection. Prosomio exists to make both easier to come by.”

The problem

Consultation practice is hard to get, and harder to get feedback on.

Most revision tools are built to test knowledge, but consulting is a performance skill. The trainees we work with tell us the same things again and again:

  1. 1

Practice partners are hard to line up.

Rotas, on-calls and family life make regular role-play sessions difficult to arrange, especially in the final months before an exam.

  2. 2

    Feedback is inconsistent.

Notes from a friend playing the patient are kind but often vague. Trainer time is precious and can't stretch to every case.

  3. 3

Question banks test the medicine, not the conversation.

You can know the guideline and still lose the consultation. You lose it through structure, timing, missed cues or a plan the patient never agreed to.

  4. 4

    Trainers are stretched.

Creating good cases and study materials, running sessions and giving individual feedback all take time that educators rarely have.

Why we built it

We built the practice room we wanted when we were training.

Prosomio was created by practising GPs and GP trainers. Between us, we have sat consultation exams and supervised trainees through them. We've also run small-group teaching on wet Wednesday afternoons when half the group was on call.

Again and again we saw capable doctors whose medicine was sound but who hadn't had enough chances to consult out loud and hear honest, structured feedback afterwards. The trainees who did best were often the ones who had found a study partner and simply put in the hours.

So we asked: what if anyone could have that? A patient to talk to at 10pm. A colleague to rehearse with, and notes to learn from afterwards. Study material that trainers can share between sessions.

That's Prosomio. It doesn't replace supervision, real patients or your training programme. It's a practice room that is always open, built by people who still work in general practice.

  1. 1

    What we saw

Strong clinicians with too few chances to rehearse the conversation itself.

  2. 2

    What we wanted

On-demand, realistic practice with feedback a trainer would recognise.

  3. 3

    What we built

One platform for solo, peer and trainer-led consultation practice.

How Prosomio works

Six ways to practise, one feedback framework.

Practise alone, with a colleague, or as part of a trainer-led session.

  • Practise with AI patients by voice or video
  • Rehearse with a colleague and review the recording
  • Face an AI viva examiner
  • Turn teaching materials into study packs
  • Build your own private cases
  • Get feedback across all three SCA domains
  1. 01

Choose — pick a case from the library or build your own.

  2. 02

Consult — practise with an AI patient, a colleague, or an AI viva examiner.

  3. 03

Reflect — read domain-by-domain feedback with specific examples from your consultation.

  4. 04

Repeat — try again, or move on to a case that stretches a different skill.

Curriculum fit

Designed around how general practice is actually assessed.

The MRCGP Simulated Consultation Assessment (SCA) consists of twelve consultations. Each one has three minutes' reading time followed by twelve minutes of consulting. Some cases are video and some are audio-only. Prosomio's AI patients work in both voice and video, so you can rehearse each format.

Each SCA case is marked in three domains. Prosomio's feedback uses the same three headings, so what you practise matches how consultations are assessed:

  • Data gathering and diagnosis

focused history, targeted information gathering, working diagnosis and red flags.

  • Clinical management and medical complexity

safe, realistic plans, shared decision-making, follow-up and safety-netting.

  • Relating to others

rapport, ideas, concerns and expectations, clear explanation, and ethical, person-centred communication.

The RCGP selects SCA cases from a curriculum-mapped bank, using a blueprint of twelve clinical experience groups. We use these groups to guide how we write and organise cases:

  • Children and young people
  • Gender, reproductive and sexual health
  • Long-term conditions, cancer and disability
  • Older adults, frailty and end of life
  • Mental health and addiction
  • Urgent and unscheduled care
  • Health disadvantage and vulnerabilities
  • Ethnicity, culture, diversity and inclusivity
  • New undifferentiated presentations
  • Prescribing
  • Investigations and results
  • Professional conversations and dilemmas

Prosomio is an independent platform. It is not affiliated with, or endorsed by, the Royal College of General Practitioners. Our cases are original and are not taken from the SCA. Our feedback is a learning aid and not an official mark.

Our principles

What we will and won't do.

  1. Clinically grounded cases

Cases are written and reviewed by practising GPs and GP trainers, and grounded in current UK practice and guidance. We review content as guidance changes.

  2. Honest feedback, no promises

Our feedback is designed to be specific, balanced and useful, and it can be wrong. We don't promise pass rates, exam results or clinical outcomes, and you won't see us claim them. Prosomio is practice, not prediction.

  3. UK data protection by design

We handle personal data in line with UK GDPR and the Data Protection Act 2018. Recordings are made only with participants' consent. Practice cases are fictional, so please never enter real patient information.

  4. Original and fair

Every case is our own work. We don't reproduce real exam material, and we would never offer an unfair advantage in an assessment.

  5. Supporting educators, not replacing them

Prosomio is designed to sit alongside supervision, tutorials and workplace-based assessment, not to stand in for them.

FAQ

Questions, answered.

Is Prosomio affiliated with the RCGP?

No. Prosomio is independent. Our feedback uses the three published SCA domains, but it is not an RCGP mark and does not predict your exam result.

Will Prosomio help me pass the SCA?

We can't promise that, and no one honestly can. Prosomio gives you more chances to practise consultations and structured feedback to reflect on. How you use that practice, alongside your training, is what matters.

How accurate is the feedback?

It's a learning aid, not an official mark, and it can miss nuance or get things wrong. Treat it as one perspective and discuss it with your trainer or study partner.

Who writes the cases?

Cases are created and reviewed by practising GPs and GP trainers. Trainers and programmes can also write their own custom cases.

Do I need a study partner?

No. You can practise entirely on your own. If you do have a partner, you can rehearse together and review the session afterwards.

Can I use real patient cases?

Please don't enter identifiable patient information. If you want to practise a scenario based on real experience, fictionalise and anonymise it fully first.

What happens to my recordings and data?

We process personal data in line with UK GDPR. Sessions are recorded only with participants' consent.

Is it only for the SCA?

No. The SCA is a common reason trainees use Prosomio, but medical students, foundation doctors and trainers use consultation practice for teaching and learning more broadly.
